Step-by-Step Instruction to Schedule Dr. Jefferson’s Consultation Service
-  Submit the patient’s records as per the orthodontic record requirement for consultation service. This includes facial photographs, dental photographs, and a pretreatment cephalometric x-ray. Poor photographs and poor X-rays will not be accepted.
- Read carefully the consultation informed consent and liability release. Please sign and submit along with the records. They can be emailed to: Drjefferson@craniofacialmedicine.com

Do not submit the patient questionnaire and disclosure of consultation fee at this time!
Dr. Jefferson will first look over the records. If the records are of poor quality, or the case is too complicated, or if the patient has unrealistic expectations, or proper forms are not reviewed and signed, Dr. Jefferson will not accept your case.
Once Dr. Jefferson accepts your case for his consultation service, you must fill out and sign the patient questionnaire and disclosure of consultation fee. Before a remote consultation appointment is made, Dr. Jefferson must receive his consultation fee. The fees are as follows:
- General public: $200 up to one hour, then $100 for each additional hour.Payments can be made by credit or debit card through Godaddy Payments, or check by mail. Mail check payable to:

These records can be obtained from orthodontists and orthodontic departments in dental schools.
Dr. Jefferson will not offer consultation service unless adequate records are submitted.

Prior to Booking
Please do not schedule an online consultation with Dr. Jefferson until Orthodontic records (facial and dental photographs and pre-treatment Cephalometric X-ray), all requested forms which are the buttons below are reviewed and signed, all required fees submitted and until Dr. Jefferson contacts you to book an appointment.
